Job Summary:
The Case Manager will have primary responsibility for assigned consumer cases and for the
implementation of those consumers’ individual service plans. The Case Manager is responsible
for providing developmental skills training for assigned consumers which includes training in
self-care, self-preservation, receptive and expressive communication, self direction, activities of
daily living, hygiene, grooming and first aid. The Case Manager should be proactive and must
focus on consumer advocacy, empowerment, and community integration.
Essential Functions:
1. Creating and implementing a comprehensive written therapeutic service plan (ISP)
for all assigned consumers and to complete all documentation which supports such
plan
2. Develop, implement and monitor interesting, creative, therapeutic goals with
individuals.
3. Complete daily data, monthly progress notes and quarterly reviews.
4. Attend trainings as assigned; maintain necessary certifications (CPR/First Aid,
SOLVE).
5. Advocate for the rights of the consumers; ensure that consumers are treated with
dignity and respect; uphold human rights of all consumers.
6. Provide skills training and support to the consumers as identified in the ISP.
7. Provide a safe environment for the consumers.
8. Assist consumers in self-advocacy, decision- making and empowerment.
9. Foster community integration and provide support to consumers to actualize this goal.
Actively seek out integrated activities.
10. Provide case management for consumers in the program as identified by supervisor,
advocate for all consumers.
11. Participate in weekly staff meetings, supervision and clinical meetings, as specified
by supervisor.
12. Maintain documentation/record keeping in accordance with agency/DMR/DMA
regulations.
13. Report significant issues (health, behavioral, psychiatric etc.) to the appropriate
supervisor in a timely fashion. Document issues/incidents according to agency
guidelines.
14. Perform personal care.
15. Perform all duties in accordance with the agency’s policies and procedures.

Qualification/Education/Experience:
1. Bachelor’s degree in a human services related field including psychology, education,
special education, recreation therapy, adaptive physical education, rehabilitation
counseling, social work or sociology and one year of experience in a training program for
the mentally retarded or the developmentally disabled. At least two years of
demonstrated, related experience in case management and developing and implementing
teaching strategies may be substituted for a degree.
2. Demonstrate strong oral and written skills (English proficiency) with consumers,
families, funding sources, consultants, the community at large, program and
administrative team.
3. Demonstrate an ability to work as a team and perform job duties with minimal
supervisory intervention.
4. Must hold a valid drivers’ license. Must have access to an operational and insured
vehicle and be willing to use it to transport consumers.
